Home & Community-Based Residential Elder Care Services

ASO Home and Community Based Adult Services offers a variety of Long Term Care (LTC) services including both Home and Community Based Services (HCBS). Our comprehensive elder care services and resources meet a unique residential and clinical needs of individuals entrusted to our care, including but not limited to the following approved waiver programs:

  • Aging Waiver
  • Attendant Care Waiver
  • Act 150 Program
  • COMMCARE Wavier
  • OBRA
  • Individuals who need significant-to-moderate support for their daily living needs; and
  • Adults who can live relatively independently in the community with staff assistance.

At ASO, our home health services and community programs are designed to promote individual confidence, independence and community integration.

Funding Options

Funding for our elder care programs come from several public waiver programs and private insurance sources.

Public funding streams include:

  • Intermediate Care Facility for individuals with Intellectual Disabilities (ICF/IID) community and campus based programs)
  • Consolidated Waiver programs
  • Person/Family Directed Support Waiver
  • Out-of-State Referrals

Private funding streams include:

  • Private pay resources
  • Trusts
  • Insurance/Legal settlements
  • Other financial models
